G Altebrando LLC dba Source Healing and Wellness (“we,” “us,” or “our”), Vermont.

 

2. Information Collected

- Personal identifiers and contact details (name, email, billing address, phone). 

- Health, lifestyle and wellness information you provide in intake and sessions. 

- Payment information processed by a third‑party payment processor (we do not store full card data). 

- Session notes, appointment records, and any documents you upload or share. 

- Technical data (IP address, device, browser) when using web or telehealth tools.

 

3. Use of Information

We use your information to:

- Provide and manage consultations, follow‑ups, and written materials. 

- Process payments and send invoices/receipts. 

- Communicate scheduling, administrative messages, and required legal notices. 

- Comply with legal obligations and protect safety (see Disclosures). 

- Improve services and maintain internal records.

 

4. Legal Basis & Consent

By engaging our Services and signing the Agreement, you consent to collection and use of your personal and health information as described. For minors, parent/guardian consent is required.

 

5. Confidentiality & Disclosures

We maintain confidentiality of your information during and after the Term, except:

- When you provide written consent (email acceptable). 

- If required by law (e.g., suspected child abuse, imminent harm to self/others). 

- For minors under 18, we may disclose to parent/guardian in situations described in the Agreement (habitual substance abuse, process addictions, referral follow‑up) or where we reasonably believe disclosure is in the minor’s best interest. 

- To third‑party service providers (e.g., payment processor, teleconferencing) for purposes of service delivery; such providers are contractually bound to protect data.

 

6. Data Sharing & Third Parties

We share data only with authorized third parties necessary for operations (payment processors, scheduling and telehealth platforms). We do not sell personal data.

 

7. Data Security & Retention

We implement reasonable administrative, technical, and physical safeguards. We retain records for as long as necessary to provide services and meet legal obligations. Requests for deletion will be honored to the extent not in conflict with legal or business recordkeeping obligations.

 

8. Your Rights

You may request access, correction, or restriction of your personal information by contacting us at sourceholisticvt@gmail.com. For minors, parent/guardian may exercise these rights on the minor’s behalf.

 

9. International Transfers

If data is processed or stored outside Vermont/USA by third parties, we will ensure appropriate safeguards are in place.

 

10. Changes

We may update this policy. Material changes will be communicated via email.
